Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

The Stata files to construct indicators and tables are now on our GitHub site: https://github.com/DHSProgram/DHS-Indicators-Stata. The code to construct perinatal mortality and child mortality is in Chapter 8. Perinatal mortality can only be calculated for children born in the past 5 years. Other mortality rates can be calculated for all births in the BR file. The FGC variables, g1*, are in the IR, BR, and KR files. You should be able to do all of this with the BR file. If you want to relate child survival to FGC, be sure to include other covariates as controls...
