Dear Colleagues,

The Demographic and Health Surveys (DHS) Program, in collaboration with USAID, is in the process of reviewing the Core/Model Questionnaires used for the Program's flagship Demographic and Health Surveys.

To this end, we are seeking input on all the DHS Core/Model Questionnaires and optional special topic questionnaire modules that were last reviewed in 2009 and are currently in use by the Program. You are invited to contribute to this review by providing input on questionnaire content.

All input is to be submitted via the Questionnaire Review portal on the DHS Program User Forum:

http://userforum.dhsprogram.com/

The Questionnaire Review portal will be open from March 6 - April 4, 2014.

Guidelines for providing input are in a READ ME folder on the User Forum, which we strongly recommend you to review prior to submitting suggestions. You will also find all questionnaires and modules being reviewed in the READ ME folder on the User Forum.

Registration is not required to read content on the Forum, but is required to post to the Forum (please see the READ ME folder).

After reviewing all input, the questionnaires will be revised and submitted to USAID for approval.

Final decisions on questionnaire and module content will be made by USAID.
