Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

This is a 1987 survey. Are you sure you want to use it?

The urban/rural classification in DHS surveys is always based on the country's census classification. The four categories in this survey must have been based on the population estimates from the most recent census, rather than on a fixed urban/rural classification. I suggest that you construct a dichotomy from those four categories. We have no information that could improve on that approach.
