@ Sonny your query sums up a major gap in literature on assessment of MNCH service quality. I have so far come across two publications that constructed a form of index; (structural & proccess indices). I have listed the papers below:

A composite measure similar to the composite coverage index or co-coverage index widely used to measure MNCH indicator coverage needs to be developed. I believe by mapping all items surveyed in all the available service quality surveys (SPA, SARA, SDI...) and looking at the WHO QoC framework, it may be possible to develop a viable composite index for quality assessment. Am keen on working on this subject.
