Please find attached recommendations for maternal health questions on transportation and inter-facility referral during labor and delivery. We submit these questions for consideration in the core questionnaire or in the optional, more detailed Maternal Health module.

There are three files attached:
1. Completed application describing the rationale and associated indicators.
2. The proposed questions inserted into a suggested skip pattern within the DHS-7 questionnaire.
3. A sample table showing how a few of the questions could be presented in a DHS report.
