Diarrhea care-seeking variables missing labels [message #16671]

The variables that specify where care was sought for children with diarrhea are missing labels so I cannot identify the type of health facility a caregiver has gone to. Specifically, variables h12b-h12d and h12m-h12w. Could someone please clarify which health care facility, provider, etc. each of these variables refers to?
