Following is a response from Senior DHS SPA Specialist, Hamdy Moussa.

Thank you for your interest in SPA data use.

In all SPA surveys, there is a separate question to identify Urban/Rural facility types in the facility identification data. The question is located in the inventory questionnaire (Q008) in the facility identification section.
