Following is a response from DHS Senior Sampling Specialist, Mahmoud Elkasabi:

As stated in the DHS reports between 1989 and 1998, the surveys were national in scope but all three districts in North Eastern Province and four other northern districts (Samburu and Turkana in Rift Valley Province and Isiolo and Marsabit in Eastern Province) were excluded from the three surveys. The excluded areas account for less than 4 percent of Kenya's population.

You can find these information under the "Survey organization" section in pages 3 and 4 in the 1989 report and 1993 report respectively. In the 1998 report, you can find it under the "sample design" section in page 6.
