Following is a response from Senior DHS Stata Specialist, Tom Pullum:

These questions are only asked about the most recent birth. If, say, you open BDKR61FL.dta in Stata and enter "tab m70 bidx, m", you will see that all of the NA cases are for births with bidx>1.
