Following is a response from Senior DHS Specialist, Noureddine Abderrahim:

Health insurance variables are V481, V481A through V841H, and V481X. However, these variables are not set for all survey phases. If the questions on health insurance are present in the questionnaire, but not as recode variables, this means that they are country specific variables. Country Specific variables start with the prefix S followed by the question number.

You need to check the data first to see if these variables (V481...) exist. If the variables exist but have no data, then the questions were not asked. If on the other hand the questions were asked but no standard recode variables, you need to review the questionnaire to get the question numbers. With the question numbers in hand, you'll be able to find the country specific variables, as explained above.

The questionnaires are included in the final report of the survey, which can be downloaded from the www.DHSprogram.com website.
