Following is a response from DHS Senior Sampling Specialist, Mahmoud Elkasabi:

Please review the instructions in the report and the github page carefully.
Both A_h and M_h are district level data; the numbers should be disaggregated by districts. In your code you aggregated the numbers across all districts and used national number. This is not correct.
