Following is a response from DHS Research & Data Analysis Director, Tom Pullum:
The main issue here is that this survey was limited to ever-married women. The actual number of women age 15-19 in the household survey (weighted with hv005) is about 4398, but only 728 of them (unweighted) were married and were interviewed. If you look at the Guide to DHS Statistics, it describes the procedure to inflate the sample of ever-married women to all women with all-women factors. The procedure involves an assumption that never-married women had no births or pregnancies.
