Hi -- Can anyone point me to a source where I can see which countries' DHS questionnaires are using full birth histories (FBH+) plus asking women to answer additional questions on pregnancy losses to document stillbirths and those that have switched to obtaining a woman's full pregnancy history (FPH) to obtain stillbirth rates?
