These questions are important and relevant for various elements (e.g., FP, MCH, other health behaviors), and should be added in the Section 10 Other Health Issues. These questions will replace question 113 in the survey.

Attached are questions related to religious affiliation and health promotion. In many of the developing countries, Africa in particular, the social sector is primarily comprised of religious denominations whose tentacles reach from urban centers to deep into rural areas and hard to reach location. The EPCMD and also PRH are exploring ways to work with these groups to promote positive behavior change.

Therefore, I am suggesting that you consider the following series of questions related to church affiliation, attendance, and health promotion messages. This information could be utilized to provide insightful information to USAID about ways in which churches are or are not effectively influencing healthy behaviors among their membership. If this question is added now, once we begin working intentionally with religious groups this data will provide a good baseline. Also, having information about frequency of church attendance will provide additional information for secondary analyses.

Due to the nature and potential bias of the questions, they should be asked at the end of the questionnaire. The way that some of the questions are worded they may introduce bias - use of words such as discouragement or encouragement) However, they need to be there to ensure that we are capturing the positive influence of religious leaders.
