I'm trying to understand how vaccination coverage rates are estimated (looking specifically at the Nigeria DHS 2013). If I understand correctly, vaccination cards are used when available and, when not available, mother's recall is used. However, in Table 10.2, there is data reported for "Vaccinated by 12 months of age" with a footnote that says: "For children whose information is based on the mother's report, the proportion of vaccinations given during the first year of life is assumed to be the same as for children with a written record of vaccination."

What exactly does this mean? I interpret this to mean that this figure uses the cards and assumes whatever rate the cards give you is assumed to be the population-level rate for all children. But I can tell from the data reported that this interpretation is not correct. Can you please advise?
