I wanted to find out how exactly the weight and height measurements for children under the age of five were taken? Were they taken with clothing on or off, for the babies were diapers allowed or not? Was height taken with shoes on or not?

Dear User,
children are measured without the shoes. Diapers may or may not be removed, but diapers are not widely used in most countries in which DHS surveys are conducted.
From the Biomarker Manual:

Ask the adult to undress the child just before taking his/her weight. Leave underpants on the child.
