I've been looking at the state-level information available, and haven't seen any separate files for Chhattisgarh, Jharkhand, Telangana, and Uttarakhand.
Were they included in the surveys at any point, and if so, where could I find that data?
Is Telangana included in the Andhra Pradesh files (APXXXXXX)?

It depends which NFHS you are referring to. Chhattisgarh, Jharkhand and Uttarakhand were only established as states in 2000, and Telangana was only established in 2014. All of the areas that eventually became part of the four new states were included in all four NFHS surveys. However, separate data files are not available for NFHS-1 and NFHS-2. For NFHS-3, Chhattisgarh is state code 22 in HV024, Uttarakhand is state code 5, and Jharkhand is state code 20. For NFHS-2, Chhattisgarh is Region 1 in Madhya Pradesh, Uttarakhand (formerly Uttaranchal) is Region 1 in Uttar Pradesh plus the Haridwar district of Region 2 (District code 13),and Jharkhand is Region 3 in Bihar.

If you use IPUMS-DHS (www.idhsdata.org), there is a geography variable that harmonizes regions across survey years. It is called GEO_IA1992_2005. You will find it under TOPICS -> GEOGRAPHY -> Integrated Geography.

IPUMS-DHS is original DHS data harmonized. It allows you to easily download a single integrated file if you are interested in viewing surveys across years or across countries. Currently, it includes the data from the IR, KR, and BR files for most African countries and India. Men, household members, and several more south Asian countries will be added by Summer 2018.
