Can you tell me what definition is used to define rural versus urban - is it a population cutoff level and does it vary by country in how it is defined in the survey?

Here is a response from one of our DHS Sampling experts Ruilin Ren, that should answer your question.

The DHS surveys never define the urban-rural in a surveyed country. We always adopt the country's urban-rural definition. So the urban-rural definition is never addressed in a DHS report. The urban-rural definition is country specific, they may vary from country to country. Some are population size based, some are infrastructure based. You may need to check the definition for a specific country from the country's statistical office or government website.
