Hello,
I am working on the Madagascar DHS of 2009. I could not find anywhere in the report nor in the methodology a reference that
specify if the index child is the youngest child under 5 years of age in the household.
Could you make some clarifications about that please?

Sorry - you will have to explain what you mean. Many tabulations in the report are for all children, some are for the last child born in the last 5 years, and some are for the youngest child born in the last x years and living with the mother. You will need to check the title of the tables to see what the unit of analysis is for each table.
