Following is a response from Senior DHS staff member, Tom Pullum:

The strata for this survey (hv023 or v023) are the combinations of county and urban/rural residence. DHS surveys are designed so that stratum-level estimates can be made. That's why there are so many county-level tables in the final report.

However, I recommend that you give standard errors or confidence intervals for estimates of rates, means, etc. I think you will see that for most indicators, the confidence intervals are pretty wide. You should be cautious about ranking the counties or claiming that, for example, there are differences between counties. Before doing that, you should do a test and check statistical significance.
