Please help how to make this table for BDHS 2014 data using Binary logistic regression.

Following is response from DHS Research & Data Analysis Director, Tom Pullum:

What is the source of the table that you took a picture of? I do not believe it came from a DHS publication. DHS does not use logit regression to estimate the infant mortality rate. You will have to ask the person(s) who prepared that table.
