A concentration index can be applied to many indicators. http://siteresources.worldbank.org/INTPAH/Resources/Publicat ions/459843-1195594469249/HealthEquityCh8.pdf provides information on the concentration index. We do not have any code for applying it to DHS data.

For the anthropometric z-scores, I suggest looking at the WHO Child Growth Standards at http://www.who.int/childgrowth/standards/en/. The Z-scores themseleves are already calculated in the DHS datasets.
