I have done a similar analysis on some Kenya data before. If you have a census of health facilities in the country with a geographic/administrative boundary identifier, I don't see why you wouldn't be able to merge these data spatially. After all, if you look at other data that collect information from households, communities, and health facilities, such as the World Bank Living Standards Surveys data (and other surveys modeled after it, such as the Indonesia Family Life Survey), the sampling strategy for the facilities data aren't as rigorous as they are for sampling individual households. Often, the sampling frame come from the list of facilities identified by community leaders and household survey respondents, rather than an official listing of facilities. The most important thing is understanding where your data came from and the potential limitations of those data.
