Following is response from DHS Research & Data Analysis Director, Tom Pullum:

In Stata, you use the following lines in the KR or BR file to construct this outcome. You also need to specify svyset and svy.

gen nn_death=0
replace nn_death=1 if b6<128

logit nn_death [predictor variables]
