Following is a response from DHS Stata Specialist, Rebecca Winter:

The issue is simply that the variable label for h46a was truncated. The complete label for this variable is "place first sought treatment for fever or cough". To look just at the place where treatment was first sought for children with recent fever, you can restrict the tabulation of h46a to children with recent fever (h22==1) and you will find 824 (unweighted) children.
