Dear User,
Thank you for your query, but the Demographic and Health Program has never collected this type of information for Indonesia. The questionnaire process is a collaborative effort between the country and other partners. You can check our website for the content of the questionnaires. I am assuming you are interested in the DHS 2012 survey. The final reports on our website contain the questionnaires in the Appendix at the back of the report. If you have additional queries, please do not hesitate to post again.
