All SPA datasets prior to 2009 did not include geographic identifying data below the sampling region including rural/urban classification. In many cases actually it doesn't seem that urban/rural classification was necessarily used to create the sample selection, though in some cases it might have been taking into account.

Surveys since 2009 including Namibia and Kenya do have GPS datasets that could be used to classify a facility as urban or rural using some other data sources on populated places.

Unfortunately there are no other sources of the urban or rural classification for those dataset prior to 2009.
