Following is a response from Senior DHS staff member, Tom Pullum:

Subsampling is done in many surveys to reduce the cost of data collection--while maintaining a sample size with sufficient statistical power. The biomarkers are the most expensive parts of a survey.

The selection of the subsample is described in the bottom paragraph on page 2 of the final report on this survey. It involved selecting alternate households for the long and short versions of the household questionnaire and alternate respondents for the biomarkers, within the subsample of households with the long questionnaire. (1/2 x 1/2 = 1/4).

In virtually all DHS surveys, the age range for women is 15-49. This is the age range for virtually all fertility and all contraceptive use.
