Following is a response from DHS staff member, Tom Pullum:
Under-5 mortality rates are calculated with the BR file, which includes children who have died as well as children who have survived. You certainly cannot calculate mortality rates with a file that omits the children who have died. You may have to go back to your merging procedure and find where it was that you dropped the children who died and put them back into the file.
Children who have died will not be in the PR file (they have b16=.) and it will be impossible to link them with their father in the MR file. Maybe the easiest solution is just to do a separate analysis of mortality with the full BR file.
