Following is a response from Senior DHS staff member, Tom Pullum:

Yes, there are 4 cases (out of more than a million) with a combination of head type and sex that should not occur. I saw them too but decided not to worry about them. It is possible that they come from a household with multiple wives or a household that has a transgender head or spouse (in this survey, hv104 could take the value 3, for transgender). It would be possible to identify the household or households in which the "illegal" combination occurs, and recode or drop them, but I did not do that. You can certainly recode or drop them if you want.
