Following is a response from Senior DHS staff member, Tom Pullum:

I recommend that you use the KR file, in which births in the past five years are the cases. It includes children who have died. Each birth has a variable "bidx". The most recent birth has bidx=1, the next most recent has bidx=2, etc. The children you want are those with bidx=1.
