Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

The Ghana Maternal Health Surveys in 2007 and 2017 were special surveys, not standard DHS surveys, and they do not have standard birth histories and IR recode files. There must have been some special procedures to estimate the fertility rates for the reports.

Ghana has had many standard DHS surveys, and you can get the fertility rates for them using TFR2 or DHS.rates or our GitHub programs (https://github.com/DHSProgram).
