Please use the following definitions for your queries:
"Percentage of facilities offering ANC where ANC services are offered on indicated days^1" in table 6.1 (Report Page-83)
Please use Q1401 and recode as 1-19 = 1 (Provides, but not everyday) and 20-31 = 2 (Provides everyday);

"Urine protein^2" in table 6.3 (Report Page-86)
if Q837B(1)=1 or Q1406(2)=1 or Q1420(4)=1

"Combined iron and folic acid" in table 6.4 (Report Page-87)
if (Q1422(3)=1 or (Q210=1 & Q906(4)=1)) for Combined iron and folic acid
