There are quite a few questions in the DHS related to how various diseases are transmitted, for example, HIV, Tuberculosis, and Malaria. Those seem like useful tools to evaluate health literacy. Showing variables available in these areas in the IPUMS-DHS database, see:
For Tuberculosis: https://www.idhsdata.org/idhs-action/variables/group?id=tb_t rans
For HIV: https://www.idhsdata.org/idhs-action/variables/group?id=hiv_ trans
For malaria: https://www.idhsdata.org/idhs-action/variables/group?id=mal_ trans

There are other more specific variables, such as whether the respondent is familiar with ORS to treat diarrhea and whether she knows how to prepare ORS: https://www.idhsdata.org/idhs-action/variables/group/mater_o rs
And there are questions about beliefs about when a sick child needs treatment: https://www.idhsdata.org/idhs-action/variables/group/mater_b elief

Depending on your interests and the countries you are studying, these give you quite a lot of possible material on health literacy.
