Following is a response from Senior DHS Stata Specialist, Tom Pullum:
If you use the KR file you will have all children born in the past five years. Children with b5=1 are still alive and children with b4=0 have died. Age at death is given by b6 and (recoded) b7. Note that if age at death was 2 years or more, the values of b7 are (usually) recoded as 24, 36, or 48 months. These values should not be interpreted literally as months of age, at death, but as recoded values of 2, 3, and 4 years. Note that you will not match with the under-five death rates, and you are ignoring censoring, but you can analyze the determinants of under-five mortality with b5 and b7.
