Please refer to The Guide to DHS Statistics http://www.measuredhs.com/pubs/pdf/DHSG1/Guide_to_DHS_Statis tics_29Oct2012_DHSG1.pdf. The publication is bookmarked. The "Fertility" boomark is the the first bookmark under "III. Guide to Individual Statistics". This section in the manual goes over the calculations which are needed using CMC (century month codes). If this has not answered your question adequately, please reply.
