All the cluster level and individual level data is available in the DHS main datasets, these are available for download after registration from this website:
https://dhsprogram.com/data/dataset_admin/login_main.cfm?log out=&CFID=142516&CFTOKEN=1ff6fc91d11aeed1-812CFCEB-A 19E-2578-0408FC76D8476C60

You need to be able to use a statistical software to create and compute the variables yourself.

The geolocated information (latitude and longitude) used to create the modeled surfaces are also available from that data request website.
