Following is a response from Senior DHS staff member, Tom Pullum:

That is an issue between you and the journal. DHS always recommends the use of weights. Otherwise, estimates are biased toward the over-sampled strata, which tend to be the smaller strata. That's true for all statistics, not just for means.
