Following is a response from DHS staff member, Tom Pullum:

The data files include height and weight and anthropometry scores for children in the PR and KR files, as is standard for DHS surveys that include anthropometry.

The extra questions just ask whether the child was previously measured, in the past 3 months, by any healthcare provider or community health worker. These are yes/no questions and not measurements. The measurements in the data were done on the day of the household interview following normal DHS procedures.
