Following is a response from DHS staff member, Tom Pullum:

Unfortunately, the two sets of questions were asked in different subsamples. If you enter "tab v042 v044" you will see that there was no overlap in the subsample for the DV module and the subsample for hemoglobin testing (which included many other variables besides hemoglobin). This kind of design is found in many DHS surveys. The intention is to reduce the length of the interview and the burden on the respondents, but it means that certain combinations of variables cannot be analyzed together. There's really no way to get around it.
