Appendix A in Volume 2 of the NFHS-5 national report describes the sample design for NFHS-5. At the time of the 2011 Indian Census, there were only 640 districts in the country. Those are the 640 districts that were used in NFHS-4. The 707 districts in NFHS-5 are the 707 districts in India as on March 1, 2017. Many of the 640 districts in NFHS-4 were bifurcated or otherwise changed by March 1, 2017, so they are not the same as the 2011 Census district borders. Some state borders were also changed between the time of the 2011 Census and NFHS-5.
Districts with district codes 801-932 are not comparable between NFHS-5 and NFHS-4 or the 2011 Census, so it's best if you stick with the district codes 001-640. 575 of the 640 district codes from 001-640 are comparable between NFHS-5 and the 2011 Census. In NFHS-5, 65 districts were affected due to boundary changes and there are 132 newly created districts.
