Following is a response from Senior DHS staff member, Tom Pullum:

For the 2017-18 survey, the data for Chapter 15 of the final report, including table 15.3, were obtained in a separate "Community Questionnaire". This questionnaire is given in the final report, starting on page 445. On our website you will find "Service Availability Raw", with the Stata data in "BDSQ7RDT.zip". Let us know if you have difficulty matching table 15.3.
