Following is a response from DHS staff member Tom Pullum:

The household interview is with a specific person, the "household respondent", whose line number within the household is hv003. If you open the PR file in Stata and enter "tab hv101 hv104 if hvidx==hv003" you will get the distribution of the relationship to head and the sex of the household respondent. In most surveys the household respondent is the head or spouse of head, but it can be virtually anyone in the household who is age 15+. Typically other household members will be present and they may help, but the interviewer is not supposed to direct questions to anyone other than the household respondent. Also, when the interviewer first approaches the household, she or he (usually she) does not assign or designate who will be the household respondent.
