Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

The unique identifier for a woman in the IR file is v001 v002 v003, and the unique identifier for a child in the KR or BR files is v001 v002 v003 bidx. bidx is nested within v001 v002 v003. There can be duplicate values of bidx if the household incudes more than one woman with children.

If the child is in the PR file, its line number in the PR file (hvidx) is given in the KR or BR files with b16. You should not find any duplicates of b16 within a household. Please let us know if that is what you are seeing.
