Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

Neonatal deaths are deaths on completed days of age 0-27 (the first 28 days). You can identify them as children in the KR or BR files with b6<=127. Alternatively, people sometimes use b7=0, where b7 is a recode constructed from b6 as "0 completed months".

b7=0 includes days 28, 29, and 30, which should technically not be counted as neonatal. Even though much of the earlier analysis of neonatal mortality was based on b7=0 rather than b6<=127, I recommend that you use b6.
