Following is a response from DHS Research & Data Analysis Director, Tom Pullum:
When there is no pregnancy history, stillbirths can only be identified in previous 5 years using the contraceptive calendar (vcal_1) as a pregnancy termination (T) preceded by 6 or more months of pregnancy (P). The code to do this is in Chapter 8 of the GitHub Stata programs (https://github.com/DHSProgram/DHS-Indicators-Stata). Very few women report more than one stillbirth in the past five years (very few report even one), and we cannot go back earlier than that.
