Our Senior Data Quality Coordinator, Kia Reinis has modified the paragraph as follows:

The below excerpt from page 5 of the survey final report (available for download from our website) identifies the implementing organizations of the survey (note that your reference to ORC Macro should be changed to ICF International):

"The 2011 BDHS was conducted under the authority of the National Institute of Population Research and Training (NIPORT) of the Ministry of Health and Family Welfare. The survey was implemented by Mitra and Associates, a Bangladeshi research firm located in Dhaka. ICF International of Calverton, Maryland, USA, provided technical assistance to the project as part of its international Demographic and Health Surveys program (MEASURE DHS)."

[Source of above statement: National Institute of Population Research and Training (NIPORT), Mitra and Associates, and ICF
International. 2013. Bangladesh Demographic and Health Survey 2011. Dhaka, Bangladesh and Calverton, Maryland, USA: NIPORT, Mitra and Associates, and ICF International.]

It is true that an interview was conducted only if the respondent provided their verbal consent in response to being read an informed consent statement by the interviewer.

Whether or not the paragraph you wrote is fine for the journal would seem to be a decision between you and the journal.
