Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

DHS surveys cannot be used to calculate a crude death rate. Even if you had completely accurate sibling histories--and birth histories--you would not get a complete and representative sample of all the deaths that occurred in a reference period such as the year before the survey. If a household survey includes a question about whether there were any deaths in the household during a reference period, then there is a possibility of developing an estimate, but even then there will be issues. I'm sure there is good documentation for how the official statistics in India were constructed.
