Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

I followed up on the woman in BF6. She had three children in the past five years (v208=3) but they all died before the date of the survey. The Stata lines that are copied below show this.

You could confirm this just within the IR file. For example, for the youngest child, b5_01 (in the IR file) is 0, indicating that the youngest child had died. If this child had b5_01=1, that is, had survived, you would look at b9_01, which is 0 if the child is alive and living with the mother. If b9_01 was some other numeric value, the child would be alive and not with the mother. I suspect your other examples will also show that the children have died or, if alive, are not living with the mother. DHS does many consistency checks with these variables.
