Following is a response from DHS Research & Data Analysis Director, Tom Pullum:

The full id for a child depends on what you want to do with it. If you want to merge with the PR file, the id is v001 v002 v003 b16 (b16 is the line number in the household file, but not all children are in the household survey). Otherwise, the id is v001 v002 v003 bidx (bidx is the sequence number in the birth history, with bidx=1 for the most recent birth; every child has a unique value of bidx for the mother whose id is v001 v002 v003).
